My entry implies my willingness to obey all of May Fair Arts Festival rules if accepted. I will be present at May Fair and my booth will be open and manned during all hours, each day of the event. I understand that I may not begin dismantling my tent or display until after closing on the final day of May Fair. I take full responsibility for the safety of my equipment and myself. In using my own tent, I will take whatever means necessary t o secure my booth to my satisfaction for the protection of my own work. I will not hold Assistance League of Norman, the City of Norman or its staff responsible in the event of loss or injury and will do my best to secure my items and tent due to the possibility of inclement weather. I have read all of the information provided in this brochure and application and agree to all terms and conditions set forth. I understand that acceptance is a commitment to show and that once notified of acceptance, no refunds would be given after March 1. I agree to comply with all policies, rules and regulations regarding May Fair Arts Festival.

INDEMNIFICATION AGREEMENT

My signature below indicates that in consideration of being selected to participate as an exhibitor, vendor, or entertainer at the May Fair Arts Festival, I agree to fully release, indemnify and hold harmless Assistance

League® of Norman, the City of Norman, and their

officers, agents, employees, independent contractors, and all persons, natural or corporate, in privity with them, against all claims, damages

or other causes of action, including but not limited to, personal injury, death, property damage, lawsuits or judgments made by or against me, including court costs, expenses, and attorney’s fees, and all other expenses, which are associated directly or indirectly with the activities, events, or programs of the May Fair Arts Festival.

I further agree that this release and indemnification shall apply whether or not the claims, damages or other causes of action, including but not limited to personal injury, death, property damage, lawsuits or judgments made by or against me, including court costs, expenses and attorney’s fees, arise directly or indirectly from the negligence, action or failure to act, whether willful, intentional or unintentional, of Assistance League of Norman, the City of Norman, or their officers, agents, employees, independent

contractors, invitees, licensees or guests, or any vendor, exhibitor, or entertainer, their agent, or any other persons, natural or corporate in privity with Assistance League of Norman or the City of Norman.

Artist Checklist (initial):
