Agreement

I have read the application instructions and vendor requirements and agree to their terms and conditions. I certify that all work submitted is my own original creation and that images and descriptions are representative of my artwork. I agree to hold Phoenix Festival of the Arts and Phoenix Center for the Arts harmless for any loss, damage, or injury resulting from my participation in this event.
I assume all responsibility for my artwork and display. I agree to follow the
 rules and decisions of festival management should the festival close because of inclement weather or any other reason. I agree that jury decisions are final.
I will allow the festival to use images in promotional materials and to use photos and videos taken during the festival for promotional purposes this year and in future years. 
I will abide by setup and breakdown rules.
 I understand that festival management has the right to disqualify work that doesn’t meet festival standards and that such work will be removed from the display immediately.
 I understand that failure to comply with any of the requirements may mean removal from the festival and/or not being allowed to participate in the future.
 I understand that festival management may revise the regulations and may terminate the agreement, at its sole discretion. I will abide by the decisions and regulations of the Phoenix Festival of the Arts Committee. I release the Phoenix Center for the Arts, the Phoenix Festival of the Arts and the City of Phoenix from any loss or damage to my person or property or that of my associates while at the Phoenix Festival of the Arts. I understand the application fee is non-refundable.
