It is understood that the Bellevue Festival of the Arts is under the sponsorship of the Craft Cooperative of the Northwest (CCNW).

I /We hereby release and hold harmless the Craft Cooperative of the Northwest, it's officers, directors, coordinators, employees, volunteers and agents from any and all liability including, but not limited to: theft, personal injury, bodily injury, strike, public enemy or act of God and to indemnify it for any damages arising from my conduct at the event. This includes any liability that may arise out of the acts and/or negligence of the participant, or the participant's employees or other persons acting on behalf of the participants. Participant further agrees to hold CCNW, it's officers, directors, coordinators, employees, volunteers and agents harmless from and against any and all product liability claims which may arise out of the sale of products by the participant at the festival. Participant understands that failure to comply with the Bellevue Festival of the Arts rules, regulations and posted code of conduct could result in removal from the festival. Participant understands that removal is at the discretion of the Festival Directors and Production Managers, CCNW and it’s entities will not be held responsible for any loss of income or expenses related to removal, including but not limited to travel expenses, booth fees, or food and lodging.

I/We have read and agree to abide by the Bellevue Festival of the Arts rules and regulations enclosed with this registration.

I /We agree to abide by the decision of the panel of jurors as to the acceptability of my application and that this application does not guarantee a space.

I/We agree to allow the Bellevue Festival of the Arts use of enclosed digital images and other promotional materials (including photographs and videotapes) taken during the Festival for purposes of promoting the Festival this year and future years.

I/We understand that all participants are required to carry $1,000,000 in general liability insurance with the Craft Cooperative of the Northwest named as additional insured on their policies. Should I be accepted to the 2018 Bellevue Festival of the Arts, I will provide a certificate of liability insurance for July 26th-July 29th, 2018 (4 days total) in order to be assigned a booth space.

Participant has read and understands the above legal agreement. Participant has also read and understands the rules and regulations section as well as the provided booth information and load-in/load-out procedures, and agrees to abide by the rules, regulations and parameters set within this application.
