I AGREE TO THE FOLLOWING:

   I will carry the necessary insurance coverage for pictures, photographs, art objects, jewelry and other property displayed by the exhibitor at Excelsior Art on the Lake 2019, on Friday-Sunday, June 7-9, 2019.

   
   


2. 
   

   I will only sell items that were juried into the show.

   
   


3. 
   

   I have created all the work on display and I accept the rules and regulations regarding participation in Excelsior Art on the Lake 2019.

   
   


4. 
   

   I will provide officials at Excelsior Art on the Lake with proof of General Liability Insurance and a ST19 form by June 1, 2019 - email here otherwise I will not be allowed to set up on June 8 until I do so.

   
   


5. 
   

   Excelsior Art on the Lake 2019 has my permission to use my digital art for show advertising and/or publicity.
