Crooked Tree Outdoor Art Fair 2019 - Traverse City - 59th Annual Agreement I agree to release Crooked Tree Arts Center (CTAC), Northwestern Michigan College (NMC)/Grand Traverse Parks and Recreation, their respective boards, employees, volunteers, and sponsors from any and all loss, damage, injury, costs and expenses, and/or liability and claims resulting directly or indirectly from participation in the 59th Annual Crooked Tree Outdoor Art Fair. I understand that CTAC and NMC/GRand Traverse Parks and Recreation do not carry insurance to cover artwork and/or personal property at the Crooked Tree Outdoor Art Fair.  As an independent contractor, I understand that I am advised to carry my own insurance policy. I understand that a Michigan tax ID number is required to participate in the Crooked Tree Outdoor Art Fair, and that I am responsible for all state sales taxes and any other legal requirements associated with my participation and sales at the Art Fair. I understand that, if two or more people collaborate to produce the artwork, each collaborator must be listed on the application. I certify that I am the artist(s) who create(s) the work I sell, and am not a representative(s) or wholesaler(s).  I understand that I must be present for the entire duration of the Art Fair, and that I may not send agents, dealers, or representatives in my place, including spouses, friends, or relatives. I certify that all work displayed is handcrafted and created by me.  I understand that work created from commercial molds or kits, mass-produced work, and other such items may not be displayed, and that the display of such may result in my expulsion from the Art Fair. I agree that my booth(s) will be set up by 8:30 a.m. on Saturday, July 28, with my vehicle(s) moved to the artist parking area.  Furthermore, I understand that I am responsible for the setup, take down, and appropriate cleaning of my booth and booth area. Should any problems arise, I agree to immediately communicate them to the Crooked Tree staff on-site. I agree to act in a professional manner, and understand that failure to abide by any of the Art Fair rules or regulations, whether in the application or created thereafter by the committee, or that causing problems for fellow exhibitors or patrons, may result in expulsion from the Art Fair, including ineligibility for judging and awards, and my immediate removal from the Art Fair.  Expulsion from the Art Fair is without the necessity of any written notice, and without legal recourse or right to rebate any portion of the fees paid. I understand that there will be no refund of fees paid, regardless of circumstance. I grant permission to CTAC to reprint/reproduce images, video, names, media, and other information for promotional use of the CTAC Outdoor Art Fair.  This includes use in press releases, print, television, CTAC website, social media, and more, in promotion of the Art Fair and/or CTAC - past, present, or future. I certify that I am of legal age and that I have read and fully understand the terms outlined above, as well as the terms outlined in the rules and regulations.
