I hereby release and forever discharge, Delray Beach Arts, Inc., Festival Management Group, all sponsoring organizations and their directors, officers, employees, agents and volunteers from any responsibility, personal liability, claims, loss or damage arising out of my participation in the show.  If accepted, Delray Beach Arts, Inc. (South Florida Garlic Fest) reserves the right to use artist’s artwork through the images submitted for promotional purposes, including advertising, publicity, marketing, web-based distribution outlets including social media for the show. No compensation will be given for use of any images. I agree to abide by all payment and cancellation schedules. I understand that the application fee and deposits are non-refundable once accepted.  I further consent to the enforcement of all event rules as set forth in the application Rules & Regulations and in subsequent correspondence.
